New Patient

Welcome to Cupertino Physical Therapy, Inc (CPT) and thank you for choosing us as your provider for physical therapy services. In order for us to assist you in achieving your return to the highest level of function possible, it is important that you consistently attend your scheduled rehabilitation sessions. This will ensure that you receive the maximum benefit from your physical therapy.

For your convenience and to minimize your wait upon arrival, our forms can be completed prior to your visit.

Appointments are scheduled on a half hour bases. We ask that you arrive on time for your appointments so we are able to maintain these schedules and meet all of our clients’ needs. Your treatment will be consistently delivered by licensed therapist. On your initial visit, your diagnosis and treatment plan will be explained in detail and any questions will be answered.       

Treatment is initiated after a thorough assessment to determine an appropriate program for the diagnosis and current physical findings.                                                         

At CPT,  the patient and the therapist review progress at each appointment in order to make necessary adjustments to the treatment and home programs. The therapist communicates a treatment plan to the patient’s referring doctor and follows with progress reports to keep the doctor apprised of the patient’s improvement.

When necessary, the therapist may contact the doctor to communicate the patient’s condition or discuss other available treatment options. These communication practices are covered in the clinics HIPAA policies and procedures.

All therapists of CPT are licensed by the state and practice within the scope of the California state’s practice act.
